Parents/Students applying for a FREE instrument should use the contact form below for the initial contact. Once we receive the information in the form, we will contact you for further information. Such information may include a driver’s license, home address (license and home address should match), cell phone, name of school, church or organization, (addresses) and phone number, name of instrument and teacher at the school.
